How to qualify for an Arizona MMJ card
According to the Arizona Medical Marijuana Act, you may be approved for a medical card in Arizona if you have one of the following qualifying conditions:

Medical Marijuana Recommendation
ADHS requires an uploaded copy of your Physician Certification Form (from Leafwell). You will also need to complete a Medical Marijuana Patient Attestation via electronic signature or PDF upload.

Proof of Identity
Acceptable forms of ID include an Arizona state ID card or driver’s license.
The following are also accepted with two proofs of Arizona residency: AZ Registry Identification Card, U.S. Passport, or Arizona state identification card or driver’s license and one of the following: Birth certificate verifying U.S. citizenship, U.S. Certificate of Naturalization, U.S. Certificate of Citizenship.

Getting Your Arizona Medical Marijuana Card
- Create an account with the Arizona Department of Health Services (ADHS) Individual Licensing Portal.
- Set your password using the temporary link sent to your email.
- Log into the Licensing portal, then select the Individual Portal tile.
- Select the Add a new license tile, and then select the Marijuana tile.
- Select New Patient Application.
- Review the agreement to receive notices from the department before clicking Agree and Proceed.
- Complete the Identification Information section.
- Enter your First and Last name exactly as it appears in the electronic copy of your photo identification.
- A valid mailing address must be provided. P.O. Box will not be accepted as a residential address.
- Enter Residence Address information.
- Review the ‘Consent to Sign electronically‘ agreement. If you do not consent to sign electronically, you will need to upload a signed Medical Marijuana Patient Attestation form.
- Place your signature in the box and then click Accept.
- Caregiver Information: select response – If yes, complete available fields with Caregiver information.
- Select a response in the Request to Cultivate section, and then click Save & Continue.
- Enter Physician’s Information. The Physician Information section must match the signed physician certification form.
- Enter Qualifying Health Condition, and then click Save & Continue.
- View Other Information Section.
- Check the appropriate box for Clinical Studies.
- Check the appropriate box for SNAP Eligibility. If yes is selected and SNAP Eligibility documentation is not valid or current, ADHS will send a notification to pay the remaining application amount before the application can be approved.
- Select Save & Continue.
- Upload all required supporting documentation indicated by the red asterisk (*).
- Note: If you provided an electronic signature, you are not required to upload an electronic copy of the Patient attestation.
- Upload SNAP documentation if applicable.
- Click Save & Continue.
- Review the Application sections before continuing to payment.
- Once application data is verified, click Submit & Go to Payment to proceed to enter payment information.
- Once the payment process is completed, the Payment Confirmation page will be displayed.
- Once successfully submitted through the ADHS online system, ADHS will review your application.
- If approved, you will receive an automatically generated email indicating your approval.
- If more information is needed or the application has deficiencies, you will be notified and provided with further instructions on correcting and resubmitting your application.
- Upon approval, your registry identification card will be electronically available within the Individual Licensing Portal. You may download the card to your mobile device or print it out.

How to renew your AZ MMJ card
It’s easy to renew your Arizona medical marijuana card with Leafwell. You’ll go through most of the same steps as when you apply for medical marijuana treatment for the first time in Arizona. The only difference is that you won’t get an email prompt from the state since you already have an account in their system.
- Sign in to your Leafwell account or register as a renewal patient here.
- Get approved and receive a new, current Physician Certification.
- Complete the renewal process via your existing ADHS Individual Licensing Portal account within 90 days.*
- They require an uploaded copy of your new Physician Certification (from Leafwell). You will also need to update your Medical Marijuana Patient Attestation via electronic signature or PDF upload.
- ADHS will review your application and process the approval, within 15 business days.
- You’ll receive an email from ADHS when your electronic card is accessible on the state’s Individual Licensing Portal.

- Select a response in the Request to Cultivate section, and then click Save & Continue.

Legal limitations for card holders
Navigating the legal landscape of medical marijuana can be a bit tricky, but we’re here to break it down for you.
Semi-Synthetic Hemp-Derived Cannabinoids Illegal: Delta-8, delta-10, and other psychoactive, hemp-derived cannabinoids are illegal in Arizona. Their use, possession, sale, distribution, and production are prohibited in the state due to concerns about the unregulated market and minors’ use of these products.
Recreational Use Legal: Recreational marijuana is legal for adult use in Arizona. Recreational users, 21 or older, can possess up to 1 ounce (28 g) of marijuana (with no more than 5 grams being marijuana concentrate) or grow up to six marijuana plants at their home (with up to 12 marijuana plants in households with two or more adult members).
Home Cultivation is Allowed: Approved medical marijuana patients or caregivers can grow marijuana at home if they live more than 25 miles from a dispensary and were granted cultivation rights on their application.
If you’re 21 or older, you can grow up to six plants at home for personal use. If two or more adults live in the same residence, a maximum of 12 plants is allowed. Cultivation must be in a secure, enclosed area, such as a closet or room, to prevent access by minors.
How to become a caregiver
If you’re looking to become a caregiver for a medical marijuana patient in Arizona, there are some important rules and steps you’ll need to follow.
Picking Up for Patients
Good news! As a caregiver, you can pick up medicinal cannabis from a dispensary on behalf of a patient. Handy, right?
Caregiver Limitations
An individual may not serve as a Personal Caregiver for more than five patients at once. If the patient is a minor, having a caregiver is a must.
Application Process
Caregivers will need a completed physician’s attestation form, and they’ll require their completed application form, too. The process for a caregiver’s medical cannabis card is similar to the process for a patient.
Eligibility Criteria
If you have designated a caregiver within the ADHS online system, your caregiver must also apply.
- Your patient application must be completed and approved before your designated caregiver can apply through the ADHS online system.
- Your caregiver will need your qualified patient registry identification card number, full name, and date of birth to begin the application process.
- Please note that your caregiver must also submit fingerprints to ADHS through the U.S. Mail as part of his or her application.
- Caregiver application fee: $200

Can I use a medical card from another state in Arizona?
Arizona has medical cannabis reciprocity laws with some limitations. To buy cannabis with an out-of-state medical card in Arizona, you must qualify as a “visiting qualifying patient.”
This means you must have an out-of-state medical cannabis card with a medical condition that satisfies Arizona’s medical cannabis requirements.
And if you’re an Arizona resident traveling elsewhere, the following states and U.S. territories also accept or recognize out-of-state medical marijuana cards:

Minor patient information
Leafwell is currently unable to certify minor patients. Minors must see (2) providers. The state requires that the following (2) forms be completed by (2) different providers:
General Minor Information
- Summary: The custodial parent or legal guardian of a patient under 18 can apply for a medical marijuana card online. The patient and the parent or legal guardian must apply together and all information is entered into the same application.
